Visits are paid when you book. Ask for a billing sheet, and Dr. Berkel sends it at the end of the visit for you to submit to your insurer for reimbursement.

Visit fees
- New Patient Assessment
- $425, paid when you book, office or video. Booked as 45 minutes; with everything included it usually runs an hour. Lab work, prescriptions, forms and follow-up visits are separate.
- Annual Physical & Immigration Exam
- $425, paid when you book. New Jersey only.
- Mobile IV at home
- Adds $99 to the IV visit.
- House call
- A house call adds a mobile service fee, shown when you book.
- Every other visit
- The price shows when you book.
Laboratory charges
Laboratories bill separately for testing. Coverage and what you owe depend on the laboratory and on your plan.

Do you take my insurance?
Visits are paid when you book. Ask for a billing sheet, and Dr. Berkel sends it at the end of the visit for you to submit to your insurer for reimbursement. The practice does not bill insurance for these visits. Whether your plan pays you back depends on your plan.

How much is a first visit?

What if I need to cancel?
If you cancel, the full amount becomes a credit toward a future visit, good for one year. Visits are not refunded.

What does a mobile IV cost?

Will I get a bill later that I was not expecting?
The visit is paid when you book. Lab work is billed by the laboratory, not by this practice, so a blood test can produce a second bill.
