The immigration medical exam

The examination USCIS requires with a green card application, in the Paramus office, New Jersey only. She is a designated civil surgeon, so the form is completed and sealed by her. $425, paid when you book.

What the visit is

A physical examination, a review of your vaccination record, and the laboratory testing the form requires. She completes Form I-693, signs it and seals it.

It is usually two visits rather than one, because the laboratory results have to come back before she can sign.

The sealed envelope

If you are filing on paper, the envelope goes to USCIS unopened. If you are filing online, USCIS asks you to open it yourself and upload the form, and to keep the envelope until they decide.

What to bring

Photo identification, your vaccination records, and the form itself if you already have it. Do not sign it before the visit.
